Internal refactoring and structural improvements

• Internal implementations of LFS and LSS revised to ensure type-stable outputs.

• Severity, informativity, and cumulative distribution components refactored to provide clearer, more consistent internal data structures.

• shannon() now returns a tidy data.frame representation to simplify downstream use and improving consistency.

• Cumulative frequency calculations (eq5dcf()) revised to construct a cumulative distribution based on relative frequencies. Percentage-based summaries retained for presentation.

• Informational measures such as the Health State Density Index (hsdi()) now operate explicitly on proportional cumulative distributions with rounding confined to presentation contexts.

• Tests updated to reflect the updated code changes.

• Shiny application logic updated to align with the revised code.

DSU mapping

• Updated DSU mapping lookup tables to include the UK EQ-5D value set (UK_2026). The original DSU mapping value set is now explicitly available as England_2018, with country = "UK" retained as a backwards-compatible alias. Additional DSU mappings for other countries are also available.

• Utility values in DSU lookup tables are now stored rounded to 3 decimal places and copula mapping values rounded to 8 decimal places, improving consistency with standard EQ-5D reporting, reducing floating-point matching issues, and decreasing package size.
